W-9 Form Requirement
The USAFL requires all umpires to complete and submit an IRS Form W-9 before receiving any payments. This ensures the League remains compliant with US Federal and IRS regulations.
Important
You will not be able to receive payment for Regionals or Nationals until your W-9 is received.
How to Submit
- Download the official 2024 IRS W-9 form: Download W-9 (IRS.gov)
- Complete the fillable PDF
- Email the completed form to treasurer@usafl.com
